Manor Solomon's stoppage-time winner secured the Championship title for Leeds United as they won 2-1 at Plymouth Argyle, whose relegation was confirmed.
Burnley's 3-1 win over Millwall gave Leeds the title on goal difference, having secured promotion back to the Premier League last month after two seasons away.
Argyle took the lead when the impressive Mustapha Bundu's shot rebounded off the post and into the path of Sam Byram, moments after the Plymouth striker had struck the Leeds upright.
Leeds, top of the table going into the final day, were frustrated time and time again by a dogged Plymouth side until early in the second half when Willy Gnonto headed Solomon's cross wide.
The visitors had Joel Piroe's goal ruled out for offside 17 minutes from time as Daniel Farke's side searched for the winner they knew they needed to secure the title.
But just when it looked as if they would have to settle for second place, Solomon cut in from the left and unleashed a low shot into the bottom corner to send the travelling Leeds fans into a frenzy and spark a celebration after the final whistle.
Leeds United winger Manor Solomon speaking to BBC Radio Leeds:
"I can't put into words what I'm feeling right now. It is a dream come true to score the winning goal and take this championship. Throughout the season we've shown we deserve this trophy and to finish in the first spot. I'm so glad it happened.
